Initial questionnaire

Filling out the preliminary questionnaire gives the clinical psychologist an idea of your past development and current situation. This will help you understand how you are feeling and what support you need to find the best solutions to support your wellbeing.

You can fill out a questionnaire before your first appointment or during a meeting with a psychologist.

The answers will help the psychologist better understand the nature of your problems and find the right approach.

 

1. Early development: Record or study information about your childhood – newborn, toddler, adolescent development, and adult experiences. Did you experience any difficulties?

2. Development of the problem: how long does the current problem last? Have any critical events in your life or in the lives of your family members affected her?

3. Goals: What do you want to solve?

4. Obstacles: What prevents you from achieving the desired result?

5. Significant circumstances: Is there anything that a clinical psychologist needs to take into account (physical or mental health factors)?

6. Support Network: Who are the people close to you who can support and help you?

7. Strengths and resources: What are your strengths that can help you overcome challenges?

8. Therapeutic expectations: What will help you feel that therapy has achieved its goals?
